Introduction:
This document is a legally binding agreement that affects the legal rights of your minor child(ren) and yourself. By signing it, you and your child(ren) give up certain legal rights, including the right to sue. Please read carefully.

Section 1: Acknowledgment of Risks
I, {contact_name}, as the parent/legal guardian of {name}, acknowledge that Brazilian Jiu-Jitsu (BJJ) and related activities conducted by Labyrinth Brazilian Jiu-Jitsu LLC carry inherent risks, including but not limited to physical contact injuries, strains, fractures, cardiovascular stress, and the potential for severe, permanent injury or death. These risks may result from my child’s actions or inactions, those of others, or the condition of the facility.

Section 2: Assumption of Risk
I willingly agree to assume all risks associated with my child’s participation in BJJ activities, including those risks arising from the negligence of Labyrinth Brazilian Jiu-Jitsu LLC and its employees or agents.

Section 3: Release and Waiver of Liability
I hereby release, waive, discharge, and covenant not to sue Labyrinth Brazilian Jiu-Jitsu LLC, its officers, agents, or employees (“Releasees”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or death that may be sustained by my child, or to any property belonging to my child, while participating in BJJ activities. Additionally, I agree not to sue the Releasees on my own behalf regarding any claims arising from my child's participation in BJJ activities.

Section 4: Indemnification
I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs they may incur due to my child’s participation in BJJ activities, whether caused by the negligence of the Releasees or otherwise.

Section 5: Acknowledgment of Health Risks (Including COVID-19)
I understand that participation in BJJ activities during the COVID-19 pandemic or other infectious diseases presents additional risks. I affirm that my child is participating voluntarily and have taken all necessary precautions. I release the academy from all liability for any illness, infection, or health complication that may arise from their participation.

Section 6: Consent to Medical Treatment
I consent to medical treatment for my child in case of an injury, accident, or illness during their participation in BJJ activities.

Section 7: Acknowledgment and Understanding
I have read this Waiver and Release of Liability, fully understand its terms, and understand that I am giving up substantial rights on behalf of my child and myself, including the right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
